Inclusive Schools Week Nomination Form

Nominate YOUR "Inclusivity Hero" by November 30 for him/her to receive a gift card to Rock n Joe's and a recognition certificate
(see example below).

Gift card and recognition certificate will be personalized and mailed to your nominee's place of employment (i.e. school, office, etc.), so please provide that mailing address - not your personal mailing address.  Your nominee will receive the items and a note letting them know that you were the person who submitted the nomination.
