This event is a DSAF event organized and sponsored by two parents of a child with Down syndrome, Jamie Bertucci, and Maranda Meece. DSAF will seek to have a 2:1 ratio of children to high school/college volunteers. Jamie and Maranda will also be onsite the entire time. Young adult self-advocates will be in attendance as well to assist with supervising the activities with the children.
Please review the parental waiver below. You will be expected to agree to the waiver during the registration process to sign your child up for the event.
PARENTAL WAIVER: I hereby acknowledge that a Parents Night Out event was provided on February 15, 2020 from 4pm-7pm, upon parent and/or legal guardian request, to individuals over age 18 months to 15 years. I hereby request to have my child participate in the event, and in consideration for my request:
I HEREBY RELEASE DOWN SYNDROME ASSOCIATION FOR FAMILIES OF NEBRASKA (“DSAF”) and its officers, agents, employees, and volunteers, from any and all claims, potential claims, charges, damages, losses, injuries, whether personal, bodily, economic or non-economic, related to, concerning, or arising from or during the Parents Night Out event.
I HEREBY INDEMNIFY DSAF and its officers, agents, employees, and volunteers, from any and all claims, potential claims, charges, damages, losses, injuries, whether personal, bodily, economic or, non-economic, that may be brought, related to, concerning, or arising from or during the Parents Night Out event.
FURTHER, I ALSO AUTHORIZE DSAF to use any photographs or video of me, my minor children, and my family, taken at the Parents Night Out event in advertisements or other publications.
During the registration process, please check that you agree to this waiver to sign your child up for the event. Those children whose parents who do not desire to complete the waiver will not be allowed to participate in the event.
